  • COMMUNITY NURSING SERVICE

    The Community Nursing team provides a broad range of nursing expertise within either the home/work environment or a clinic setting. Our highly skilled nurses focus on  a holistic approach to the patient’s needs and have the ability to work alongside the patient/whanau and care givers to implement a package of treatment that promotes health and wellness. Total care health services has over the years established collaborative relationships with key stakeholders, practitioners, nurses and multidisciplinary teams to ensure that prompt evidenced-based  care can be provided to achieve positive patient outcomes.

    Community Nursing Services encompass:

  • Wound management
  • Medication administration and intravenous therapy
  • Pain management
  • Nutrition and feeding
  • Bowel and bladder care
  • Continence management
  • Monitoring and maintenance of skin integrity
  • Phyiscal mobility and function improvement
  • Discharge planning

  • The nursing service works collaboratively with other health providers to ensure cohesion and continuity of care. Of paramount importance is the Community Nursing Service’s ability to identify and meet the needs of ethnic communities and disadvantaged groups. The geographical area covers Warkworth to Hamilton, including all areas of Auckland. Referrals are processed and services commenced within 24 hours.

    Members of this team effectively plan, direct, coordinate, and oversee the delivery of community nursing services. Our Nursing Administrators are responsible for ensuring that our Nursing Service clients receive the treatment and support required within stringent timeframes so that healing and recovery are maximised.
